Value-based care is the predominant model for enabling the healthcare industry to control costs and deliver better information to consumers. The basic idea is that reimbursements are based on the quality of the outcome of a procedure, episode of care, use of a device or therapy. Under this model, life sciences companies are rewarded for improving health outcomes and/or reducing the costs to achieve those outcomes. It requires life sciences companies to rethink many of their processes, from R&D through the commercial phase. Navigating those momentous shifts requires that life sciences companies embrace a range of digital technologies which will enable a holistic approach to value-based care. While 95% of hospitals in the United States have implemented Electronic Health Records or EHR, much work remains.1 Patient identification errors continue to plague hospitals and are the precipitating factor for 13% of surgical mistakes and 67% of transfusion mix-ups.2 Nationally, medication errors cause at least one death every day and injure 1.3 million people annually.3 The cost of lost lives, reduced productivity and malpractice insurance fees are debilitating the healthcare industry. At the center of every hospital’s safety program is the patient wristband, which provides caregivers access to the information they need at every point of care during the patient stay. However, not all wristbands are of equal quality. Some wristbands are not durable enough to withstand the rigors of a busy hospital environment, leading to workarounds and mistakes that, in the end, comprise patient care. H

Detecting and preventing errors that threaten patient safety is a closed-loop process that begins at the point of care, extends to independent laboratories, and then back to the caregiver. Sample identification and results reporting errors can lead to misdiagnosis and inappropriate treatment with deadly consequences. A 2006 Wall Street Journal article reported that while malpractice claims for pathology errors are relatively low, they are the second most costly. In addition to creating a serious risk to patient safety, sample misidentification creates significant financial implications. Redraws, retesting and additional treatment that result from sample errors cost the healthcare industry an estimated $200 million to $400 million per year.

There are several reasons for this, including the inability of healthcare institutions to constantly monitor post-operative deterioration as well as patient's inability to visit the healthcare center for post-operation followups. Most of the patient data is recorded manually. This data is often weaned from displays of multiple monitoring devices like vitals monitor, infusion pumps, and ventilators. More often than not, these devices do not talk to each other or to a central system. Doctors, nurses, or other care-givers manually note down patient statistics in their files and notepads. Even though the healthcare industry is in the throes of digitization, there's still a lot of paper doing the rounds. eventually, the data from these physical sheets might be fed into an electronic system, but there is a high chance of error in that process.
